Product Comments Mod

I need a mod completed asap which goes along the lines of a "product comment" type system.

Much like the "customer reviews" on this page:

I want the following information:

* Product Rating
* Review Name (title, etc)
* See all my reviews (so linked to the person's username?)
* The actual review

* Was this review helpful to you .. Yes | No

Now for the most important things.

* I want to be able to preview reviews before we post them on the site.
* I want people to be able to edit their review, but if they do edit them after we have approved them, they need to be previewed again by us.
* I want an option to be able to set how many reviews can be posted for each product.

** ... Anything else you can think of!

nb: You won't have access to our site to code the mod.. It;ll need to be done and then submitted to us for review by our web guys (who are currently snowed under)!
